• Bi-weekly schedule: 7 days on, 11 days off - then 3 days on, 7 days off

• Designed to optimize both performance at work and quality of life at home About the Role: Extended Care RNIn this rewarding after-hours, on-call role, you will:
  • Triage and manage after-hours patient needs via phone and onsite visits
  • Conduct hospice informational visits and nursing admission assessments
  • Support patients and families by collaborating on individualized Plans of Care
  • Work alongside an interdisciplinary team to educate, advocate, and provide holistic, compassionate care
  • Deliver exceptional service while making a real impact during the end-of-life journey What We Offer
