Job Description

General Purpose of Job:
As part of the Population Health Service Organization (PHSO), the Population Health Liaison will support PHSO operations by serving as the conduit between the PHSO and Physician practices within Sparrow Care Network (SCN). This position will drive awareness and adoption of Sparrow's population health management efforts and provide continuous support using various systems, tools, and education. The Population Health Liaison will play a critical role in solidifying a collaborative relationship with providers in SCN.
Common Duties and Responsibilities: The Population Health Liaison will work directly with Physicians and office staff to ensure they have the support, tools, and education to succeed in various population health initiatives and achieve organizational population health goals and objectives. It is expected that this will be achieved through the application of evidence-based best practices, data analysis, interpretation/identification of opportunities, communication and consistent follow-up and evaluation.
Essential Duties:

  • Serves as a key interface and liaison for participating Physicians of SCN,including independent Physician Practices, Sparrow Medical Group (SMG),and Sparrow Health System (SHS)
  • Participatesin practice transformation including practice observations, review offindings, best practice recommendations related to a practice's readinessfor value-based care
  • Utilizessystem databases (e.g., EPIC, CMS, MVC) to produce applicable reports. Analysesdata, maintains reports, and summarizes activities regularly
  • Routinelyanalyses clinical quality, cost, and utilization data for all providers
  • Responsiblefor supporting the operations of policies and procedures, provideroffice/billing staff education, and workflow training, including updates and maintenance on processes related to various population health initiatives
  • Maintains and delivers materials to support population health goals and objectives, to include training agendas, presentation materials, sign-in sheets, and scheduling of training sessions
  • Provides Physician office staff education regarding implementation, monitoring, and performance of chosen clinical and administrative initiatives
  • Provides coaching on pre-visit planning and closure of gaps in care
  • Facilitates consistent, accurate, and optimal documentation by providing both formal and informal educational programs around risk assessment
  • Provides one-on-one education of reports and other communications as appropriate to identify potential improvements and assist Physicians, managers and other appropriate office staff in understanding methods and processes required to achieve measurable results
  • Works closely with Physicians and team members to ensure the appropriate processes are being followed
  • Works with SCN leaders to advance efforts to standardize care among primary care Physicians through the development of quality measures and benchmarks

    * Bachelor's degree in Business, Healthcare Administration or related field and a minimum of 1-year experience in Physician practice operations and/or Management Services Organization OR * Associates degree in Business, Healthcare Administration or related field and a minimum of 3 years of experience in Physician practice operations and/or Management Services Organization.


    Specialized Knowledge and Skills
    * Demonstrated skills in the areas of written and verbal communication, judgment, problem-solving, presentation, and public relations. * Must possess a keen working knowledge of physician practice operations, procedure and diagnostic coding, and Electronic Medical Record (EMR) usage in ambulatory settings. * This position requires a dynamic and highly motivated person with the ability to inspire and lead people as well as communicate effectively with employees, physicians, other Management Services Organization administrators, staff, and hospital staff. * Demonstrate ability to empathize and communicate comfortably with people of diverse backgrounds. * Must possess exceptional communication skills to present facts and recommendations effectively in oral and written form, including accurate grammar and business correspondence knowledge. * Strong interpersonal skills to establish and maintain effective business relationships; demonstrated ability to communicate in situations requiring tact and poise; skilled in confidentiality and discretion. * Must present a neat, professional appearance. * Must be able to multi-task, and focus with composure, in accordance with changing deadlines and priorities. * Must be proficient with the following software: Microsoft Word and Excel. * Must have a working knowledge of the following software: Microsoft Access and PowerPoint. * Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job.
